Workers’ Compensation Claims Support

If you would like to start a workers' compensation claim online, please fill out an Injury Reporting Form OR call us at (800) 367-3743.

Here’s the information that you’ll need in order to submit your claim online OR when you call us:

Employer information

• Workers’ compensation policy number
• Policyholder’s name
• Company tax ID number

Injured employee information

• Personal information about the injured employee, such as name, address, phone number, social security number and date of birth
• Occupation, hire dates, current position and wage information

Incident information

• Date and time of the injury and when was the accident reported to you and by whom (name of the person)
• Type of injury, part of the body injured, cause of accident, where did the injury occur and where was the injured employee treated
• Name of witness(es)

ERIE will file all the required reports with the state workers' compensation agencies for you.

After a claim is filed, an ERIE claims adjuster will get in touch with you, the injured worker and the medical provider.

When a catastrophic or serious injury occurs, ERIE's nursing team will contact the hospital and act as a liaison between all parties throughout the claims process. Our nurses add an extra level of medical expertise to help ensure you (and your employee) receive the support that you need throughout the claims process.

Our team plays an important role in managing workers' compensation cases by:

• Helping your injured employee get medical treatment
• Working closely with you and the medical provider to coordinate your employee’s safe and timely return to work
• Working with you to identify modified and transitional duties that your employee can perform as soon as he or she is medically able

We’ll also be available to help you complete the following forms to get medical clearances from a physician and identify employee's physical restrictions:

• Job analysis form (completed by employer)
• Physical capability assessment form (completed by physician)

Prescription drug program

Injured workers will be provided with a pharmacy card, which is presented to pharmacies for prescription drugs for compensable injuries. Among its benefits, the program can help the employee avoid out of pocket expenses and reduce pharmacy costs. A mail-order option is also available and it can provide long-term medications at reduced cost and greater convenience.

The workers’ compensation laws in many states, including Pennsylvania, Kentucky, Virginia, West Virginia, Tennessee, North Carolina and Indiana, let employers establish physician panels for work-related injuries.

In Pennsylvania, employees need to sign this form: Employee’s Acknowledgement of Physician Panel (Spanish). In other states, like Wisconsin, Illinois, Maryland and the District of Columbia, voluntary panels are offered to employees, but they’re not required for work injuries. ERIE will recommend medical providers for your physician panels.

To request a physician panel, fill out this form. Please specify your policy number on the request.
